The following advice is adapted from NICE Guideline CG170: Autism in under 19s: support and management [http://www.nice.org.uk/guidance/cg170]

Young people with ASD who are receiving care from CAMHS or a child health team should be reassessed at age 14 to establish whether they will need an adult service.

If an adult service will be required, give information to the young person about services and treatment that will be needed.

Prepare for a smooth transition, which should be completed by the age of 18.

Assess the following, making use of any existing documentation:

  • Personal functioning
  • Occupational functioning
  • Educational functioning
  • Social and communication functioning
  • Any coexisting conditions, especially the following:
    • Depression
    • Anxiety
    • ADHD
    • OCD
    • Global delay or intellectual disability

For young people aged over 16 with complex or severe needs:

  • Complete a Care and Treatment Plan in accordance with the Mental Health Measure (Wales).
  • Involve the young person in the planning (and where appropriate, their parents/carers).
  •  Provide information about adult services, and their right to a social care assessment.

Consider a formal meeting involving health and social care and other relevant professionals from child and adult services.
